In this field, enter the full name and date of birth of the person to whom you have paid settlement payments.

As a rule, settlement payments are payments made as part of a pension rights adjustment or other legal obligations (e.g. after a divorce).

If you are claiming financial settlements for the first time under the law of obligations in respect of pension rights adjustment, enclose the contract or the pension agreement with your declaration.

The consent of the divorced spouse/life partner is required. Please enclose Form U.
