First-time application / change

Tick this box if you are providing information about a disability for the first time in the assessment year 2018 and have not yet submitted any proof of this to your tax office.

To provide proof of this, please submit the following documents to the tax office, if they have not already been submitted in previous years:

• Severely disabled card or
• Confirmation from the social services office or
• Confirmation from the care insurance fund about care level 4 or 5
